The Care Coordination Intake is a one-on-one conversation with a Tranquil Innovations team member to help you understand your options and find the right level of support. Whether you're navigating mental health challenges, struggling with day-to-day independence, or supporting a family member who needs help — we're here to listen and guide you forward. In this session, we will: - Talk through what's going on in your life right now - Review your current needs, circumstances, and goals - Help identify whether Community Alternative Supports (CAS), mental health services, or another pathway is the right fit - Connect you with the appropriate team or next steps This is a non-clinical service and does not include diagnosis or therapy. It's simply a supportive, no-pressure conversation to help you find your footing.
